Weeks 1–2
Pain
Calm it down first.
Mornings stop setting the tone for the whole day. Hands-on work settles the irritated nerve and the guarding around it, starting on visit one.
What We Treat
The catch when you stand up from the couch. The ache that decides whether today is a good day. The leg that has started tingling on the drive to work. It is fixable, and it starts with finding the cause instead of guessing at it.

Why It Happens
Your back doesn’t hurt for no reason. And it’s rarely just one thing. Usually it’s some mix of four: a disc that’s irritating the nerve running down your leg. Spinal joints that stiffened up and stopped sharing the load. Deep stabilizing muscles that quietly went offline after your last flare-up. And the daily habits, how you sit, bend and lift, that reload the same spot before it can heal.
None of that shows up in a quick glance at your back. It takes a cause-first evaluation to sort out which one is actually driving your pain. That’s why rest, ice and a printed stretch sheet keep coming up short. They answer the symptom, not the source. And you’re the one everybody else leans on: the driver, the lifter, the one who gets it done. Which is exactly why you’re the last one to get looked at, and exactly why you’re reading this.
The Honest Truth
Nobody regrets starting too early. Every week, someone in our clinic says they regret waiting.
How We Fix It
Every plan has an endpoint. Here’s what the four phases look like for a back and a sciatic nerve.
Weeks 1–2
Pain
Mornings stop setting the tone for the whole day. Hands-on work settles the irritated nerve and the guarding around it, starting on visit one.
Weeks 2–6
Prime
Bending, sitting and standing up stop being decisions. Targeted mobility work frees the stiff segments that have been guarding the injury.
Weeks 6–10
Perform
A long day stops costing you the evening. Progressive core and hip strengthening builds the support your spine was missing in the first place.
Weeks 10+
Prevent
Lifting, bending and twisting get tested under real load before you’re cleared. That’s an endpoint, not another appointment.
Not too long ago, I needed to get therapy for a severe case of sciatica. After receiving a detailed evaluation of my sciatica, Ajay and Minal developed a comprehension therapy program for me. After two short months of therapy my pain from the sciatica was relieved.

Phase one, Pain: settle the nerve so a morning stops setting the tone
Phase four, Prevent: lifting and bending tested before you’re cleared
Questions, Answered
Usually not, and it’s rarely the first step. Plenty of people with no back pain at all have disc bulges and wear and tear on their scans, so an image doesn’t always name the cause. We start with a cause-first evaluation to find what is actually driving your symptoms. If imaging turns out to be genuinely necessary, we’ll help you get it. You also don’t need a physician referral to begin: New Jersey has direct access, so you can call us first.
In most cases, movement helps more than rest does. Hurt doesn’t always mean harm, and total rest tends to leave a back stiffer and weaker, which sets up the next flare-up. The work is dosing the right movement for your back, not any back. Your licensed therapist sorts that out at your evaluation, then progresses it with you one-on-one, every visit.
You get a plan with an endpoint, not an open tab of appointments. Your plan runs the four phases: Pain, Prime, Perform and Prevent. At your evaluation, your licensed therapist tells you what that looks like for your back, how many visits it should take, and what has to happen to earn the next phase. We also verify your insurance benefits by phone before your first visit, so you know your cost before treatment starts.
